In Blasphemous, there are a variety of key items you'll come across. These range from essential items that you'll need to progress the story, to additional items for side quests and to unlock upgrades such as relics. Some items are essential to the main story of Blasphemous, whether they're key to the story itself or are needed in order to progress. In the table below, you'll find a list of all the essential main story items as well as information on where you can pick them up.

| Thorn | This is the very first item you'll receive when you start Blasphemous. You'll be given this by Deogracias. |
| Holy Wound of Attrition | Reward for beating Ten Piedad in the boss fight at the end of Mercy Dreams |
| Holy Wound of Contrition | Reward for beating Tres Agustias in the boss fight at the end of Grievance Ascends |
| Deformed Mask of Orestes | Reward fo beating Melquiades, The Exhumed Archbishop in the boss fight at the end of Mother of Mothers |
| Embossed Mask of Crescente | You'll pick this up from the Library of the Negated Words after defeating Exposito, Scion of Abjuration. |
| Mirrored Mask of Dolphos | In Convent of Our Lady of the Charred Visage accessible when you reach Wall of Holy Prohibitions |
Side Quest and Misc Items

| Key to the Chamber of the Eldest Brother | This can be purchased from Candelaria the Merchant in Mercy Dreams for 400 Tears of Atonement. It will give you access to the Blood Perpetuated in Sand relic. |
| Bouquet of Thyme | You can pick this up from the fast travel room in Albero, which you can access once you've reached the fast travel room in Mercy Dreams. You need this for Tirso's side quest. |
| Linen Cloth | Tirso will give you this once you've given him the Bouquet of Thyme. This is needed for the Lord of the Salty Shore's side quest. |
| Tentudia's Carnal Remains | You'll need the Blood Perpetuated in Sand to reach this. It's in Wasteland of the Buried Churches, at the bottom right of the section with the broken bridge. This is needed for Ludivicio's side quest. |
| Black Grieving Veil | In Desecrated Cistern, towards the top of the area. This is needed for the Altasgracias side quest. |
| Empty Golden Thimble | This is picked up from Gemino in Where Olive Trees Wither. You'll fill it once you beat Our lady of the Charred Visage and return it to him. |
| Incense Garlic | Picked up from Where Olive Trees Wither and needed for Tirso's side quest. |
| Remains of Tentudia's Hair | Before you reach Graveyard of the Peaks, you can pick this up from Where Olive Trees Wither. This is needed for Ludivicio's side quest. |
| Torn Bridal Ribbon | Can be purchased from Candelaria the Merchant in Graveyard of the Peaks for 800 Tears of Atonement. This is needed for the Altasgracias side quest. |
| Immaculate Bead | You'll get this from the Confessor Statue room in Graveyard of the Peaks. It's needed for changing the endings and entering the Confessor Statue arenas. |
| Golden Thimble Filled with Burning Oil | You can fill this up in the chalice of burning oil after the fight with Our Lady of the Charred Visage. This is needed for Gemino's side quest. |
| Bouquet of Rosemary | In Jondo, if you go all the way down to the bottom side on the right. You need this for Tirso's side quest. |
| Dried Clove | From Mountains of the Endless Dusk after Jondo. You'll need the Blood Perpetuated in Sand to reach it. This is needed for Tirso's side quest. |
| Dried Flowers Bathed in Tears | You can pick this up from Gemino in Where Olive Trees Wither or from the tomb in the cave to the left of Gemino. This is needed for his side quest. |
| Melted Golden Coins | In Grievance Ascends, just before the battle with Tres Angustias. Climb up to the very top of the room and you'll pick it up. This is needed for the Altasgracias side quest. |
| Egg of Deformity | From the Altasgracias at the bottom of Grievance Ascends. |
| Hatched Egg of Deformity | Take this to the Mountains of the Endless Dusk where you fight Perpetva, and hatch it in the tree hit by lightning. This is needed for the Lord of the Salty Shore's side quest. |
| Olive Seeds | In Patio of the Silent Steps, this is in the third section of the area. Climb up, then drop down on the left to pick up the item. This is needed for Tirso's side quest. |
| Sooty Garlic | Found in The Sleeping Canvases in the room with the long ladder and three enemies in pictures. You need this for Tirso's side quest. |
| Tentudia's Skeletal Remains | At the top of Graveyard of the Peaks on the right-hand side. This is needed for Ludivicio's side quest. |
| Severed Hand | You can get this from the Descerated Cistern in a secret passage you slide through opposite the room with the blood fountain. You'll need the Nail Uprooted from Dirt that you get at the end of the Redento questline in Mother of Mothers. |
| The Key of the Inquisitor | You can purchase this from Candelaria the Merchant in The Sleeping Canvases. It's needed to open special rooms in Wall of Holy Prohibitions. |
| Mark of the First Refuge | From Archcathedral Rooftops. This is needed for the Cleofas side quest. |
| Mark of the Second Refuge | From Archcathedral Rooftops. This is needed for the Cleofas side quest. |
| Mark of the Third Refuge | From Archcathedral Rooftops. This is needed for the Cleofas side quest. |
| Cord of True Burying | Return to Albero after giving Cleofas the above items, then speak to Ludiviso before you speak to Cleofas in Archcathedral Rooftops. This will safe Cleofas in his side quest. Note: to arrange the meeting between Redento and Cleofas, skip picking this item up. |
| Key of the Secular | You'll pick this up in Wall of Holy Prohibitions. It will unlock additional rooms in the area. |
| Key of the Scribe | You'll pick this up in Wall of Holy Prohibitions. It will unlock additional rooms in the area. |
| Chalice of Inverted Verses | You can get this from the Desecrated Cistern later on in the game. You'll need the Linen of Golden Thread from The Sleeping Cavases and the Three Gnarled Tongues relic to reach the room from the elevator shaft. |
| Custodia of Sin | This is the result of beating all of the Confessor Statue arenas and is needed to unlock the true endings. |
